Even as an internal inquiry by Government Multi Specialty Hospital (GMSH),Sector 16,has found certain lapses on the part of the hospital employees,the UT Administration has ordered a magisterial probe that will look into the circumstances that led to the death of a nine-month-old foetus and damage to the kidneys of the mother Suman,30,who was transfused blood meant for another patient,incidentally her name sake. The magistrate has been asked to submit the report in seven days.
I have ordered a magisterial probe which led to the death of the foetus. The probe will be held by SDM (South) Vinod Kavele, Chandigarh Health Secretary Ram Niwas told Newsline. The SDM has been told to submit the report in seven days. The probe will find the exact circumstances and people who are responsible for the negligence.
Earlier in the day,a preliminary report was submitted by GMSH to the UT Administration which admitted lapses taking place in the hospital during transfusion of blood to Suman,who is now battling kidney failure at the Postgraduate Institute of Medical Education and Research,(PGIMER).
We submitted the preliminary findings about the case to the Director (Health Services), GMSH Deputy Medical Superintendent M P Singh said. We have not specifically identified any individual for negligence but,yes,there were lapses. The lapses were on the part of the blood bank as well as those who transfused. But only an indepth inquiry can find who exactly was at fault, he added.
GMSH sources said that the officials entrusted with the job of finding the facts recorded the statements of doctors,paramedical staff and blood bank officials who were on job in various shifts during the period when Suman was admitted in the hospital through the OPD on December 16 . The sources said that the report has found serious lapses on part of the blood bank officials and paramedical staff.
The inquiry has also suggested several steps to ensure that such negligence does not recur.
Suman was transfused blood meant for another patient (her namesake) at GMSH on December 16,resulting in the death of the nine-month-old fetus and damage to her kidneys. She is now admitted at the PGI where doctors say that she might need dialysis.
Her husband Desh Raj,originally a resident of UP,lodged a formal complaint with the police on Sunday,seeking action against the guilty.
