Bengal SIR 2.0: Name deleted from voters’ list? New voter? How to apply through Form 6 with Annexure IV

The second phase of the Special Intensive Revision (SIR) of electoral rolls has begun in West Bengal. At present, electors whose names have no mapping with the 2002 electoral roll are being called for hearings.
After the conclusion of the first phase of SIR, around 58 lakh names were deleted from the electoral rolls. Following this, the Election Commission of India (ECI) has opened a fresh application window. Those whose names have been deleted or who do not figure in the 2025 electoral roll can now apply for inclusion as new voters.
To apply for fresh inclusion or re-inclusion in the electoral roll, applicants must submit Form 6 along with Annexure IV. The application can be submitted online through the ECINET app or the ECI website, or physically through the local Booth Level Officer (BLO) along with the required documents.
The applicant must fill in their first name, middle name and surname in the local language as well as in English.
The applicant must mention the name of a relative — father, mother, husband or wife. In the case of an orphan, the legal guardian’s name must be provided.
A mobile number and email ID are mandatory, along with Aadhaar card number and gender. The date of birth has to be furnished along with age proof. Accepted documents include birth certificate, Aadhaar card, PAN card, driving licence, certificates of Class X or XII board examinations, and passport.

The applicant also must provide their address with valid address proof. Accepted documents include water, electricity or gas bills (at least one year old), Aadhaar card, passbook of a nationalised bank or post office, passport, revenue or land holding records, registered rent or lease agreement, and sale deed.
Applicants with disabilities must submit a disability certificate along with a declaration specifying the percentage of disability.
Details of relatives whose names already appear in the electoral roll, including address and EPIC number, must be provided.
Details of Annexure IV
Annexure IV is largely similar to the enumeration form used during the SIR exercise.

In the form, the applicant must declare their name, mobile number, address, and the names of father, mother and spouse, along with EPIC number, if available.
The left column requires the applicant to state whether they previously had an EPIC number during the 2002 SIR.
In the right column, the applicant must mention the name of any relative whose name appeared in the 2002 SIR.
All documents will be verified by the BLO with self-attestation. In the case of new voters who have come from Bangladesh, the Citizenship Amendment Act (CAA) certificate will be accepted as proof of citizenship, as announced by the Election Commission.
